| 1. | Please enroll me in Pistol One and notify me as to the dates available. |
| 2. | At D.V.C. Shooting Sports safety is of primary concern. The only effective way of policing safe gun handling by students is to establish strict rules and enforce them. Any student who does not comply fully with D.V.C. Shooting Sport's safety policy will be expelled from that course and his/her tuition forfeited. |
| 3. | I have filled out Certification of Training form, as required by D.V.C. Shooting Sports. |
| 4. | Any prospective student under the age of eighteen, must be accompanied by their parent or guardian who must also be enrolled. |
